Objective: To identify the influence of clinical setting on adherence to antiretroviral treatment among HIV/AIDS infected adolescents in Thika Level 5 hospital. Design: Crossectional. Setting: Thika Level 5 hospital, Thika Sub County, Kiambu County. Subjects: 150 HIV/AIDS infected adolescents and 25 health Care Workers. Results: Pearson Product Moment Correlation analysis revealed that there was a significant relationship between clinical setting and respondents’ adherence to ART, (r=0.220, p<0.05 level). In line with the findings, Chung et. al. (2011) found that early intensive adherence counseling when a patient is being started on HAART resulted in a significant reduction in poor adherence and virologic failure while use of alarm device had no impact. Intensive counseling of respondents before and after ART initiation provides channels for improving communication and building rapport and trust between the clinician and respondents. It also provides the respondents with the necessary knowledge and education that is significant to adherence to ART and retention in the ART program.
